• Send us the bills for each date of service you want us to litigate
• Include whatever denials you received – or, if you don’t have them, just tell us which services are in dispute and why they were denied
• Complete our short Litigation/Arbitration Summary Form.

• Assignment of Benefits (AOB) – if available
• Medical Records – notes, reports, test results, hospital record

These extras can always be provided later if needed for discovery or trial. They are not required up front to get your case filed.
